About Rhinoplasty
A rhinoplasty is a surgical procedure that reshapes and recontours the nose to improve its aesthetic appearance and/or function. Rhinoplasty can reshape several features of the nose, including the nasal bridge, dorsal hump, nasal tip, and nostrils, leaving the face more balanced and proportionate. (1) This procedure can also correct concerns such as a deviated septum, breathing difficulties, or deformity from trauma. It is one of the most widely performed cosmetic surgical procedures in the country, with surgeons performing more than 48,000 rhinoplasties in 2024 alone. (2)
Gender-Affirming Rhinoplasty
Gender-affirming procedures help patients align their appearance with their gender identity. While the nose may seem like a minor consideration in one’s overall appearance, it is a central feature of the face and can contribute to a masculine or feminine look. There are now several new techniques that allow patients to feminize or masculinize their noses so that they can feel more comfortable and confident in their appearance.
When performing rhinoplasty as part of facial feminization or masculinization surgery, plastic surgeons focus on either angularizing or softening the features of your face. Gender-affirming rhinoplasty is highly individualized but typically consists of specialized techniques to alter the nasal tip, narrow or widen the nasal dorsum and alar base; each of which can help patients feel more aligned with their identity.
Male Rhinoplasty
Male rhinoplasty is a specialty within rhinoplasty procedures that focuses on enhancing the aesthetic appearance and function of the nose while preserving traditionally masculine features. People usually associate rhinoplasty with female patients, but male rhinoplasty is far more common than many realize, especially in cases where sports or other activities injure the nasal structure and require surgical correction.
Male nasal anatomy is slightly different from female anatomy due to gender differences in bone, cartilage, and skin. Men typically have thicker nasal bones, cartilages, and nasal skin, as well as more prominent nasal dorsum and tip, while the female nasal structure possesses smaller bones and cartilages, often appearing narrower and sharper. When performing male rhinoplasty, plastic surgeons will customize techniques to the patient’s anatomy and the type of corrections being made to ensure proportionate, natural-looking results.
Preservation Rhinoplasty
Preservation rhinoplasty represents a shift in rhinoplasty technique, as this procedure focuses on eliminating the dorsal hump while preserving nasal dorsal anatomy, in particular the connection between the upper lateral cartilages and nasal bones. In turn, patients often experience a quicker recovery and more natural-looking results.
While preservation rhinoplasty itself is not a new concept, it has become more popular over recent years as patients’ interest in achieving naturally looking results and minimizing the chance of adverse outcomes grows. When performing this procedure, the plastic surgeon works underneath the dorsal hump to modify deeper structures (mainly septal cartilage and bone) to allow for the dorsal hump to flatten. This is in contrast to standard hump reduction techniques, where the hump gets physically removed with special instruments.
Ultrasonic Rhinoplasty
Ultrasonic rhinoplasty, sometimes referred to as the Piezo technique, is a rhinoplasty method that employs advanced surgical instruments to minimize damage to the surrounding nasal structures. These tools use ultrasonic vibration technology to carefully reshape the nasal bones to address asymmetry, prominent humps, and other abnormalities or irregularities, while preserving surrounding cartilages and soft tissues. The introduction of ultrasonic rhinoplasty and Piezo technology has created new advancements within the field of rhinoplasty surgery. This technology improves the accuracy of nasal bone manipulation, reduces post-operative side effects by avoiding soft tissue and cartilage trauma, and enhances the smoothness of the final bony contours. The precision of ultrasonic rhinoplasty can produce more predictable results, enhancing patient satisfaction.
Non-Surgical Rhinoplasty
A non-surgical rhinoplasty uses dermal fillers to augment the appearance of the nose without surgery. (5) In recent years, more patients have turned to non-surgical and minimally invasive treatments to reduce their recovery time, try a new look without commitment, and achieve more natural-looking results. (2) As a result, new treatments have emerged to address milder to moderate concerns in the nose, all without requiring surgical intervention.
Dermal fillers are gel-like injectables that we administer into various areas of the face to restore volume and augment certain facial features. Dermal fillers may be injected into specific areas around the nose to smooth out the nasal bridge, elevate the nasal tip, or enhance the symmetry or straightness of the nose. Non-surgical rhinoplasty does not alter the cartilage or bony nasal structures, but it effectively adds volume to create a more aesthetically pleasing nose.
3D Imaging in Rhinoplasty
Technology has become increasingly interwoven with plastic surgery over the last several years, with one of the most recent developments being the use of 3D imaging technology. This technology helps patients visualize their results before they receive their procedure. This technology is especially helpful when performing complex procedures such as aesthetic rhinoplasty.
Traditionally, rhinoplasty planning depended on photographs and physical exams. While these are still useful and important, solely relying on this method led to discrepancies between the patient’s desired appearance and their received results. Now, patients may take several photos of their face at various angles and upload them to a simulation system. The system then generates a 3D model of the patient’s face from the submitted photographs. Through this process, you can visualize different adjustments to plan a procedure that delivers your ideal nose.
Many practices already implement 3D imaging, as these visuals can bridge the divide between the anticipated and attained results. However, this technology can sometimes lead to the development of unrealistic standards or specific expectations. This may be especially apparent when surgeons achieve results that do not align with the simulated ones. (6)
